1. I have truthfully represented to Studio XI (InkedByChloe) that I am eighteen (18) years of age or older. If I am underaged, I am aware that I will need my guardian to fill a parental consent form on the day of the appointment.

 

2. I acknowledge and understand that the time and cost quotes I was given were estimations and may

vary depending on multiple factors at my session. I will come prepared for the possibility of a longer
session. I understand that a session includes the time spent discussing, making, placing stencils, drawing the designs on skin, as well as tattooing time. 

 

3. I am responsible for planning my trip before my appointment to ensure that I will arrive on time. I will notify the artist if I will be about 15-minute late through email. If I am 30-minute late, there will be additional charges. If I am more than 45-minutes late, unless I am the only client of the day, the appointment will need to be rescheduled to a different day and another deposit will be required.

 

4. Disrespectful and rude behavior towards the artist will not be tolerated and may result in the immediate termination of the session. In such cases, the deposit will be forfeited, and no further sessions will be provided.

 

5. I acknowledge that it is not reasonably possible for the representatives and employees of Studio XI to determine whether I might have an allergic reaction to the dyes, pigments, or processes used in my tattoo, and I agree to accept the risk that such a reaction is possible.

6. I acknowledge that I have advised my tattoo artist of any condition that might affect the healing of

this tattoo. I do not have medical or skin conditions such as but not limited to: acne, scarring (Keloid),
eczema, psoriasis, freckles, moles, or sunburn in the area to be tattooed that may interfere with said
tattoo. If I have any type of infection or rash anywhere on my body, I will advise the tattoo artist.

 

7. I acknowledge that all skin is unique and a touch-up session might be needed after the initial tattoo session. I understand that a touch-up session is used to restore details fallen out during the healing process and I am subject to extra charge if I request a change of tone, colour, and additional details.

 

8. I am not pregnant, and if I am breastfeeding then I am fully aware of the potential risks, have

discussed it with my doctor, and consent to be tattooed at this time.

 

9. I will not show up at my appointment under the influence of alcohol or recreational drugs.

 

10. I acknowledge that infection is always possible as a result of obtaining a tattoo, particularly in the

event that I do not take proper care of my tattoo. I have received aftercare instructions and I agree to
follow them while my tattoo is healing. I agree that any touch-up work needed, due to my own
negligence, will be done at my own expense. This includes sun and ultraviolet frequency damage.

 

11. I acknowledge that a tattoo is a permanent change to my appearance and that no representations

have been made to me as to the ability to later change or remove my tattoo. To my knowledge, I do
not have physical, mental, or medical impairment or disability which might affect my well-being as a
direct or indirect result of my decision to have a tattoo.

 

12 I acknowledge that if my tattoo is a symbol or something written in any language, the tattoo artist and the owner of the business are not responsible for what it may or may not mean.

 

13. I acknowledge that if my tattoo includes text or number that I am responsible for checking the correct order, spelling, grammar, and punctuation. I am responsible for proofreading the text before it is applied as a tattoo.

 

14. I acknowledge that variations in color, shades, and design may exist between any tattoo as selected by me

and as ultimately applied to my body, depending on many variables including skin tone.

 

15.  I acknowledge the decision for obtaining my tattoo is my own free will and choice. I consent to the

location of the tattoo and the performance of the tattoo procedure.

 

16. I acknowledge and agree to following any and all instructions provided to me regarding the

maintenance of a sanitary environment while I am being tattooed.

 

17. I am aware that the art will be created in the moment without pre-planning.

 

18. I am familiar with the artist's styles. By booking a session, I agree to trust the process and communicate any specifications to the artist if needed. I acknowledge that the artist is not responsible for dissatisfaction with the completed tattoo. Any modifications or additions to the tattoo after the session is finished will require booking additional sessions at my own expense.

 

19. I agree to IMMEDIATELY notify the artist if I feel lightheaded, dizzy, and/or are about to faint before,

during, or after the procedure. Failure to do so releases Studio XI and my tattoo artist
of all responsibility.

20. I acknowledge that my artist has their specific artistic process, and I understand and agree that the tattoo process is artistic in nature and not clinical therapy.
